feat: Cloudflare Worker URLs und LFS fetchexclude konfiguriert

- LFS-Download-URLs in index.md auf Cloudflare Worker umgestellt
- fetchexclude für unterlagen/** in .lfsconfig hinzugefügt
- npm script 'lfs-fetch-all' zum manuellen Abrufen aller LFS-Dateien

Große Dateien werden nicht automatisch beim Clone heruntergeladen,
sind aber über Worker-URLs mit korrekten Dateinamen verfügbar.
